What are the first steps I should take?

One of the first things you should do is obtain an ACAT (Aged Care Assessment Team) assessment. It’s necessary if you wish to access government-funded services such as a residential place in one of Autumn Care’s homes. Most people are referred to an Aged Care Assessment Team by their doctor, community nurse or social worker, but this is not essential as you can contact an ACAT directly and arrange an assessment for yourself. To find out more, go to

Aged Care Australia - Being Assessed: http://www.agedcareaustralia.gov.au/internet/agedcare/publishing.nsf/Content/Being%20assessed-1.

The need to move into an Aged Care Home is not always a planned one but your decision should be well informed. The most informative place for you to start is the Aged Care Australia website shown below.  Their 'check list' of things you need to consider is comprehensive and you'll find the most common questions answered here.  If you have any questions, please do not hesitate to contact us to speak with our home managers and nurses.

Autumn Care Homes
Autumn Care has four residential aged care homes in Melbourne. We encourage direct enquiries. You can call or email the Director of Nursing at any of our facilities at any time, or book in for one of our regular facility tours. A current schedule of tours, along with contact details (telephone and email), can be found by clicking here . Alternatively, ask the social worker you are dealing with to give us a call on your behalf. Please read more about our homes here .
